Impotence And The Penis - Are Erections All In The Mind

Male impotence – or the inability to enjoy sexual intercourse due to an insufficiently rigid penis – is a huge problem, and while 1 in 10 men will experience difficulty in achieving an erection at some point in their lives, just one-tenth of those with impotence problems will seek help. Impotence can have a devastating effect on a man’s self-confidence and his relationship, but what makes the situation unnecessary is the fact that over 90 per cent of cases of impotence can be cured if help is sought. Advances in treatment have meant that over 90 per cent of impotence cases can be cured so don't put unnecessary strain on yourself or your relationship but failing to get help.     

Inaddition, 44 per cent of men with the condition suffer from depression as a result, and 35 per cent from anxiety. Many men blame impotence as the cause of a relationship break-up, and the problem is compounded by the fact that nearly two-thirds of them believe it is brought about solely by psychological problems.
The erection: a medical guide
But to understand why impotence occurs and how it can be tackled, it is necessary to know the process by which a man achieves and maintains an erection.

First of all, sexual arousal is needed, stemming from sexual thoughts and sensations. Many people, and in particular women, wrongly believe that drugs like Viagra actually spark sexual arousal. In fact, the chair of the British Association for Sexual and Relationship Therapy (BASRT), Marj Thoburn reminds us, these drugs only enable those thoughts to translate into physical results.

When the mind is aroused, it sends nerve impulses around the body to increase production of a chemical messenger that causes blood vessels in the penis to expand and fill with blood. This causes the erection. The amount of the chemical messenger is controlled by an enzyme called PDE5. However, men with erectile dysfunction need larger amounts of this chemical to achieve a worthwhile erection, so drugs like Viagra work by blocking PDE5, enabling more of the chemical to be produced.

Is An Erection Mind over matter?
Only 20 per cent of cases of impotence are psychological, in which sexual thoughts are impaired and prevent the body from releasing chemicals required for erection. This can be caused by a wide range of psychosexual worries and concerns – or simply from overwork, tiredness or stress. The failure to produce an erection can create a vicious circle in which the man becomes so anxious about becoming aroused that he is unable to do the very thing he is worried about. This, along with all forms of impotence, can place stress on a relationship if he then goes to extraordinary lengths to avoid sexual contact with his partner, including, most commonly, going to bed extremely early or extremely late.
Often the partner sees this as rejection and feels that they are not desired – which itself places further stress on the relationship. Or they can even suspect their partner is having an affair.
A psychological cause is most prevalent in young men, who suffer from performance anxiety - "Is it the right size? What should I do? Am I doing it right?" Young men tend to look over their shoulders watching their performance. The solution is to talk honestly about the problem, preferably with your partner, or with friends or a therapist.
The majority of cases, however, have a definable physical component. Impotence is associated with increasing age. While less than 10 per cent of men aged between 20 and 40 years suffer any problems, that figure rises to two-fifths in men aged between 60 and 69, and nearly two-thirds in those over 70.
Impotence is often the result of coronary heart disease, high blood pressure and diabetes, which all affect the blood vessels in the penis. Smoking, drinking and obesity may also have an effect. And medication for a wide range of conditions can also have an effect on maintaining an erection. Only very rarely is the problem down to a physical problem with the penis itself.
Finding your Mojo again
So what is the solution? For most men - 90 per cent - drugs like Viagra and Cialis work. They take effect in under an hour, and Cialis claims to work over a 24-hour period, removing pressure and allowing for more spontaneous and natural lovemaking.
Partners may be unprepared for the sudden re-awakening of their sexual life, particularly if there has been a long period of abstinence. And thirdly, the drugs may interfere with other medication, in particular those containing nitrates for conditions like angina.
For men who find the drugs don’t work, or those who can’t use them, there are other methods. Uprima is a drug that doesn’t clash with nitrates and is dissolved under the tongue. Failing that, some forms of the drug alprostadil can be injected or inserted into the urethra. The chemicals encourage the relaxation of the muscles and dilation of the arteries, causing an erection. The medication takes effect within 10 to 20 minutes, and can last for up to an hour.
Apart from drugs, vacuum pumps are very effective, especially for older men, who may not want to take additional pills, Marj Thoburn explains. They work to encourage blood into the penis by creating a vacuum round it. A ring can then be slipped onto the base of the penis, which helps maintain the erection.
And for the most serious cases, where nothing works at all, penile implants with pumps or hydraulic valves can enable the man to build and hold an erection.
Love the one you’re with
Not being able to achieve an erection can cut to the core of an individual’s psyche and will always cause some distress. Everyone involved in tackling impotence strongly recommends that if a man has a partner, they, too, should become involved in treatment for the condition. If a man attempts 'to take sexual function out of the context in which it survives and thrives', says Marj, treatment may not work. So while modern science may have produced the answer to many of the physical problems, it is worthless unless impotent men also tackle the complex psychological issues that also may surround it. And as millions of men around the world will testify, it is worth it.
